Why You're Losing Muscle on Ozempic (And What to Do About It)

GLP-1 medications are a powerful tool — but without the right support, you could be losing the wrong kind of weight.

You started a GLP-1 medication like Ozempic, Wegovy, or Mounjaro and the scale is finally moving. That's exciting. But here's something your doctor probably didn't have time to mention: a significant portion of the weight you're losing may not be fat. It may be muscle.

And for women over 40, that's a serious problem.

What the Research Actually Shows

Studies on GLP-1 medications consistently show that users lose not just fat, but lean muscle mass alongside it — in some cases up to 40% of total weight lost comes from muscle. For a woman already navigating the natural muscle loss that comes with perimenopause and aging, this can leave her weaker, slower, more fatigued, and with a metabolism that's harder to maintain long-term.

The cruel irony? Less muscle means a slower metabolism — which makes it even harder to keep the weight off when the prescription ends.

Why It Happens

GLP-1 medications work by suppressing appetite significantly. You're simply not hungry. That sounds like a win — until you realize that most women on these medications are drastically underrating their protein intake without knowing it.

Protein is the raw material your body uses to build and preserve muscle. When you're eating very little and what you are eating isn't protein-rich, your body starts breaking down muscle tissue for energy. Add in the fact that most women on GLP-1s aren't doing resistance training, and you have a perfect storm for muscle loss.

The Three Mistakes Most Women Make on GLP-1s

1. Not hitting protein targets When your appetite is suppressed, eating enough feels impossible. But your protein needs don't go away just because you're not hungry. Most women over 40 need 100-130 grams of protein per day — a target that requires intentional planning even when you feel like eating nothing.

2. Skipping strength training Cardio feels easier. Walks feel manageable. But resistance training is the single most effective tool for telling your body to preserve muscle during weight loss. Without it, your body has no reason to hold onto lean tissue.

3. Treating GLP-1s as a standalone solution The medication reduces appetite and promotes weight loss — but it doesn't teach you how to eat, how to train, or how to build the habits that make your results last. Women who treat it as a complete solution rather than a tool often find themselves back where they started within a year of stopping.

What to Do Instead

The good news is that muscle loss on GLP-1 medications is largely preventable with the right approach. Here's where to start:

Prioritize protein at every meal. Even when you're not hungry, aim to front-load your protein. Greek yogurt, eggs, cottage cheese, chicken, and protein shakes are your best friends. Aim for at least 30 grams per meal.

Start resistance training. You don't need to become a powerlifter. Two to three sessions per week of basic strength training — squats, deadlifts, rows, presses — is enough to signal to your body that muscle is worth keeping.

Work with someone who understands both sides. The lifestyle piece of GLP-1 success is complex, especially for women over 40 navigating hormonal shifts alongside medication side effects and changing nutritional needs. The Bottom Line

GLP-1 medications are one of the most exciting developments in metabolic health in decades. But they are a tool, not a complete solution. The women who get lasting results are the ones who use the medication strategically — pairing it with the right nutrition, the right training, and the right support system.
